Do I need a referral to schedule my appointment ?
Yes, a referral is required to schedule your appointment. All referrals must be faxed by your care provider to 218-520-3928.
Please also bring a copy of the referral with you to your appointment.

Please bring your insurance card, a valid ID, and any relevant medical records or referral letters.
