At Northland Neurology, we are committed to providing transparent and fair pricing for all our services.
Financial policies
Payment policies (copays, deductibles, self-pay discounts)
Copayments are required at the time of service.
- If you are unable to pay your copay at the time of your visit, we require a card on file to be charged once your insurance has processed your claim.
- If you are unable to afford your copay, please contact Alissa at 218-343-6670 or via email at [email protected].
Deductibles, coinsurance, and patient balances resulting from insurance denials (including inactive policies) must be paid by phone or mail.
Billing procedures
After your claim has been processed by your insurance company(s), we will send you a bill for any unpaid “patient responsibility” in the mail, or an emailed invoice if requested.
We reserve the right to reach out to you by phone to set up a payment plan for any balances your insurance has listed as “patient responsibility”.
If you are unable to pay for your services in full, we will work with you to set up a payment plan.

Overpayment & Refund Policy
If your insurance processing results in an overpayment for your services, the excess amount will be held as a credit on your account with our office.

Collection policies for unpaid balances
Any balances unpaid after 90 calendar days from the date of your first invoice will be placed on our “collections” list.
- We will attempt to contact you by phone or mail to obtain payment in full or set up a payment plan.
- You may contact our office at any time to arrange a payment plan. We will work with you to ensure the plan fits within your budget.
- Any balances unpaid after 30 days on our “collections” list will be sent to a collection agency, currently Advantage Collections, as a final attempt to obtain payment.
- Collection balances will not impact your credit score.
However, we reserve the right to collect unpaid balances through the Minnesota court system.
Good Faith Estimate (GFE)
In accordance with the No Surprises Act, we ensure that you are fully informed of your financial responsibilities before receiving care.

We provide a Good Faith Estimate (GFE) to any patients who have been referred to our practice by a doctor, physician assistant, or other medical professional. To request a GFE, please contact Alissa at amarshall@northlandneurology.com, or at 218-343-6670.
Please note, any estimates provided are not a guaranteed final bill, but an estimate based on the information available in your referral.
Cash Prices & Self-Pay Discount
We provide a 10% discount off of our Charge Amounts for all services when patients enter our practice as “self-pay.”
- Self-pay patients are required to provide payment at the time of service.
- We are willing to work with you to set up payments for self-pay services prior to your appointment.

No Surprises Act
Effective January 1, 2022
Under the No Surprises Act, you are protected from unexpected medical bills. This law ensures you won’t be charged more than expected for certain medical services.
If You Have Insurance:
Emergency Services (Not applicable to care received at Northland Neurology)
- You can’t be billed more than your in-network cost-sharing amount for emergency care, even if the provider or facility is out-of-network.
- Your insurance must cover emergency services without prior authorization.
Certain Non-Emergency Services (Applicable to services received at Northland Neurology)
- If you receive care at an in-network facility from an out-of-network provider, you can’t be charged more than your in-network cost-sharing amount—unless you sign a written consent agreeing to out-of-network charges.
Billing & Insurance Information
- Northland Neurology & Myology, PA reserves the right to bill you for any non-payment of services your insurance company deems “patient responsibility.”
Reasons for non-payment of services may include:
- Out-of-network denials
- Our patients are required to ensure our network status before entering our practice as a patient.
- We will do our best to ensure your insurance is accepted by our practice at the time of scheduling.
- A list of insurances we do not accept is available upon request, or viewable on our website (Insurance & payment – Northland Neurology & Myology, PA : click here)
- Any referral requirements to be seen at our practice as a part of insurance policies are the responsibility of the patient and referring physician.
- Inactive Insurance Coverage
- If you believe you have active insurance coverage, and your insurance company denies coverage due to inactive coverage, we will bill you for the services we have provided.
- We will do our best to ensure you have active insurance coverage prior to your appointment.
- Non-covered Charges
- Non-covered charges may be listed as the responsibility of Northland Neurology OR the patient.
- In the case of a non-covered charge, we will assume responsibility to appeal denials with your insurance company and will not bill for these charges unless otherwise agreed upon between the patient and Northland Neurology.
If You Are Uninsured or Paying Cash:
- You have the right to a Good Faith Estimate (GFE) of expected costs before your care.
- The estimate should include the total expected costs of medical services.
- If your final bill is more than $400 over the estimate, you can dispute the charge.
- We require payment prior to providing services; additional information is listed under the Good Faith Estimate and Pricing page.
Your Rights Include:
- Protection from balance billing (surprise bills) for emergency services.
- Protection from unexpected bills for certain non-emergency care.
- The right to a Good Faith Estimate for non-emergency services if you’re uninsured or self-
pay. - The ability to dispute excessive charges beyond your Good Faith Estimate.
